Vaccinations (e.g. Flu, Pneumonia and Tetanus)

Point of Care Ultrasound

Schedule Your Appointment Online
You can schedule appointments at Avanta Clinic at any time and from any place with an internet connection. No more waiting for the office to open, or finding time between work to call to make appointments.
When booking appointments online please remember to use your full legal name (E.g. Use Christopher instead of Chris).

To reschedule or cancel appointments, please use the patient portal or call the office.
The first appointment for a new patient is generally a Medical Visit (with applicable deductible or copay). New Patient Physicals will be reserved for people without any medical problems.

(You will not be able to book appointments for DOT Physical, Immigration Exam or other administrative exams. Please call the office to book those appointments.)

You can use the patient portal to send a non-urgent secure message. No more playing phone tag and waiting for a return phone call.

Access your medical record online
We strongly believe that everyone should have access to their health information. This will allow you to understand your plan of care, refer back to it if needed, share it with your loved ones or to take it with you to other health professionals. The following information is available in the patient portal:
Allergies
Medications
Vaccinations
Blood and imaging test results
Medical Problems, surgeries and procedures
Assessment and Plan part of office visit note - this contains your diagnoses and treatment plan
You can also complete and e-sign forms on the portal. No need to print them out!
In addition, you can download your entire medical record from the portal. You can follow the instructions in the linked pdf file.
Pay Medical Bills Online
You can check outstanding balance and pay your bills using the patient portal at any time.
If you receive a bill via "snail mail", you can still pay it online, either from a computer or your mobile device. You don't even have to sign into the patient portal, saving you one more step.

If medical bills are delinquent and sent to collections, a $50 fee will be added to the outstanding balance. In addition, the patient may be discharged from the practice.
If you have an outstanding balances for Aesthetic services, please call the office to make your payment.
