Payment Options:
Payment is due at time of service. This includes:
Co-pay or deductible
Patients on high deductible health plans will be required to make a deposit of $100 towards their deductible at the time of service.
Past dues, if any
Fees for services not covered by insurance
For your convenience we accept all credit cards and cash. Personal checks will be accepted only from patients that have established primary care with us. We do not accept personal checks for special exams (e.g. immigration or DOT exams).
You will receive a statement for all services provided by Avanta Clinic. Balances are due in full upon receipt.
Payments may be made online from the patient portal, or by calling the office at 860-430-5599.
As many people are on High Deductible Health Plans, there is confusion about what is a preventative visit (generally covered by insurance) or a medical visit. Please follow the link to understand the difference between a Preventative and Medical Visit?
List of fees charged by office but not covered by insurance:
We like to keep appointment slots open for sick patients and those that need urgent appointments. Implementing “No Show” and “Cancellation/Reschedule fee” allows us to ensure that slots are not being used inappropriately.
No Show Fee: $50
Cancellation/Rescheduling fee: $25 if you cancel/reschedule your appointment within 1 business day or 24 hours (whichever is earlier).
If you have appointment on Monday, you need to cancel on Friday (you will be charged $25 fee even if you send a portal message over the weekend)
If you have an appointment at 8:30 am Tuesday, you need to cancel when office opens on Monday morning (i.e. 24 hours before).
Please do not call the “doctor on call” over weekend to cancel or reschedule appointments.
If you need to reschedule or were not able to make it to your appointment due to an emergency, please let us know and we will be happy to waive these fees.
Other Fees:
Filling simple forms outside an Office Visit - $20
(No charge if forms are completed during the office visit)
E.g. Employment physical, handicap parking
(There may be some forms that will require $20 payment irrespective of an office visit as sometimes we have to go back and forth between different agencies to ensure the form is correct and complete)
Filling multi-page/complex forms - $50
This charge is irrespective of office visit as they take time to complete
E.g. FMLA, legal forms for conservatorship, Assisted Living Facility Admission paperwork
If medical bills are delinquent and sent to collections, a $50 fee will be added to the outstanding balance. In addition, the patient may be discharged from the practice.
