Medicare Annual Wellness Visit (AWV)

Medicare Annual Wellness Visit (AWV) is an insurance benefit provided by Medicare and Medicare Advantage plans to patients.

  • It involves screening and risk assessment appropriate for individuals older than 65

  • It requires completion of several health questionnaires for risk assessment

  • It can be performed by any licensed healthcare professional under the supervision of a provider

  • Patients do not have any out-of-pocket costs for this type of visit

  • It does not involve Evaluation and Treatment of existing or new medical problems (if medical problems are addressed during this visit, then copay/deductible applies for Medical Visit - please see this link for more information)

  • Traditional Medicare does not cover an “Annual Physical”

  • This visit type does NOT involve a physical exam

  • Medicare Advantage plans may cover both “Annual Wellness Visit” and “Annual Physical” (i.e. you can get 2 separate preventative visits per year)

Preparing for Annual Wellness Visit

Please have the following information in preparation for your visit:

  • List of medications and supplements

  • List of all doctors/specialists involved in care

  • List of all medical supplies/equipment that you use (e.g. Hearing aids, walking aids such as cane or walker, eye glasses or contacts)

  • Advance Directives documents such as Healthcare proxy or Living will

    • If you don’t have Advanced Directives, you can find more information on the CT State Government Website and download the pdf form

    • Please review the form, come prepared with any questions that you have to help you complete the form

Screening Questionnaire:

For Medicare to cover this type of visit, regulations require that we screen you for several health issues using screening questionnaires.

You will be given a paper copy of the screening questionnaire during your visit. Please plan to come in 15-20 minutes early to complete the questionnaire.
